Frequently Asked Questions

Q: Who can get their vaccine from a Balls Foods Pharmacy?
A: The COVID-19 vaccine is available for everyone aged 12 years and up. Only sites that have the Pfizer vaccine are able to vaccinate those aged 12 to 17 years and they will need a parent or guardian present to sign their consent forms at the time of the vaccine. The Moderna vaccine and the Johnson & Johnson vaccine are both only available to ages 18 years and older.

Q: What do I need to do on the day of my appointment?
A: Please bring your driver’s license or state ID and your pharmacy insurance card, including your red, white, and blue Medicare Part B card if you have one. You will need to stay in our building for 15 to 30 minutes after each dose, to make sure that you do not have an allergic reaction.

Q: How much does the vaccine cost?
A: The COVID-19 vaccines are FREE to everyone living in the USA - you will not pay anything to get your vaccine and you will not receive a bill. The pharmacy does bill to cover our costs for administering the vaccine, but you will not be charged.

Q: What if I do not have insurance?
A: If you do not have insurance, the pharmacy will ask for an identification number, preferably your social security number. We can also use a driver’s license number, visa number, or passport number.

Q: How do I schedule my second dose?
A: When you make your first dose appointment or receive a dose at the pharmacy, your second dose appointment will be confirmed for 21-28 days out from your first dose. For example, if your first dose is Monday at 2pm, your second dose will be 3-4 weeks out for Monday at 2pm. You will receive your vaccination record card with your second dose appointment date on the day of your vaccination.

If you received your first vaccine elsewhere and want to schedule a second dose at our pharmacy, you will be able to select “Dose 2” when scheduling your appointment using the site above.

Q: How do I cancel an appointment?
A: Please go to our Cancellation Request Form and submit your cancellation.
